Looking Ahead
UWNYC’s Health Equity work in the coming year will be expanding to offer partnerships with community-based organizations that provide access to non-health-related services and bring preventative health care to New Yorkers who lack access to primary care—through Connect2Care.
Partners will be equipped to provide additional services such as health screenings, vaccinations, immunizations, and social service connections for their communities. We expect this effort will raise health awareness among 5,000 New Yorkers residing in the Bronx and Harlem who are without a medical home and are vulnerable to co-morbidities, such as diabetes, cardiovascular disease, and hypertension.
Insights that Guide this Work
